DISCHARGE COORDINATOR POSITION SUMMARY

The Discharge Coordinator provides access to community programs, ensure continuity of care following release into the community, advocate for patient needs, and act as liaisons between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.


DISCHARGE COORDINATOR ESSENTIAL FUNCTIONS

  • Provides discharge planning for patients in a correctional environment, determining which community resources should be included in the discharge plan and setting up appointments for the patient in the community.
  • Accurately assesses patient's financial resources, makes referrals to community groups to provide assistance with medical and mental health care as needed and assists with plans for housing.
  • Documents discharge plans in medical record, completing final documentation within two working days of discharge.
  • Functions as a member of a multidisciplinary case management team.
  • Develops and maintains positive relationships with community resources.
  • Participates in the client satisfaction survey (QAPI).
  • Participates in continuing education programs to maintain skills and social work licensure.
  • Attends training and meetings as required.

DISCHARGE COORDINATOR MINIMUM REQUIREMENTS

  • A bachelor's degree in social work or related field
  • At least one year of experience in the social services field
  • Case Management experience
  • Applicants with experience in a correctional health care setting will be given preference.
  • Familiarity with medical/MH terminology/diagnosis should be required
  • Knowledge of accessing services in community
  • Licensing preferred, not required. 
  • Baseline understanding the medical parole process preferred, not required. 
  • Minimum of one year employment in a social work capacity in a public or private agency or equivalent.
  • Must receive a satisfactory background investigation report.
