Are you passionate about making a difference in people's lives? Sol Recovery Community, Inc. is seeking a dedicated Case Manager to join our team! This role is essential in supporting clients on their journey to recovery by connecting them with resources, coordinating referrals, and ensuring a seamless intake process. Additionally, this position includes on-site case management services through our supported living program.
Key Responsibilities:
  • Guide clients and families through the intake and referral process.
  • Ensure completion of intake documentation.
  • Develop and maintain relationships with community partners.
  • Meet with clients individually to assess and address their needs.
  • Provide on-site case management services for supported living clients.
  • Connect clients to vital community resources and higher levels of care.
  • Coordinate referrals to residential and detox facilities.
  • Observe and document urinalysis testing as required.
  • Maintain detailed case management notes in compliance with SRC policies.
  • Collaborate with the clinical treatment team and attend weekly case staff meetings.
  • Uphold confidentiality and ethical standards.
  • Assist with community outreach and presentations.
  • Perform other job-related duties as assigned.
Requirements:
  • Must be at least 18 years of age.
  • Social Service Worker license (SSW) or Case Management Certification (or willingness to obtain).
  • If not already certified, the case manager will be supported in completing Domestic Violence (DV) Advocacy Training to ensure they are equipped to work with this population.
  • Clean criminal record (no felonies or related misdemeanors per licensing standards).
  • Must complete a TB screening and test only if indicated.
  • Ability to pass post-offer and random drug screenings.
  • Strong computer skills, including Google Workspace, data management, and email.
  • Reliable and punctual with excellent communication skills.
  • Ability to read, write, and communicate fluently in English.
Work Environment:
  • Primarily an office-based role with some on-site case management responsibilities.
  • Requires the ability to sit for extended periods, communicate effectively, and visually observe UA testing.
