Job description

Synergy Care is looking for a qualified Licensed Physical Therapy Assistant for an open position at Concordia Life Plan Community!. We are looking for someone who will provide quality patient care and be a team player.

This position offers Independent, Assisted/Memory Care  and SNF. 
Synergy Care offers the following benefits, 12 days PTO, 3 sick days, medical/dental/vision insurance, long and short term disability, life insurance, 401K with match and free CEU's through Relias. Sign on possible for right candidate!
In order to be considered for this position you must be a graduate of an accredited school for Physical Therapy/Physical Therapist Assistant or will graduate from such an institution within the next six (6) months.
