PROVAIL has an exciting opportunity for a Community Inclusion Coordinator to join our team! In this role you will assist and support clients individualized skill development, independent living and community integration so individuals can learn how to actively and independently engage in their local community.
This is a full-time position that typically works Monday-Friday during normal business hours but may require some need for occasional nights/weekends-based business needs.

PROVAIL was founded in 1942 by parents of children with cerebral palsy. They created some of the first community services for individuals with cerebral palsy and other complex support needs in the area. Our mission is supporting people with disabilities to fulfill their life choices. We help people decide how they want to live their life in the community and then provide the support services they need to act on those choices. We are one of the state's largest private, multi-service agencies dedicated to meeting the needs of children and adults with all types of disabilities.

What you'll do:
Person-Centered Planning and Documentation (5%):
  • Assist clients in developing and maintaining individualized community inclusion plans using a person-centered approach.
  • Complete and submit required documentation, including semi-annual progress reports, in accordance with program and regulatory requirements.
  • Ensure all services and supports align with each client's community inclusion plan, goals, interests, strengths, skills, and preferred activities.

Community Inclusion and Skill Development (80%):
  • Provide community inclusion services and supports to individuals or small groups of up to three clients with similar interests or goals.
  • Support clients in identifying, accessing, and initially engaging in activities that promote socialization, education, recreation, and personal growth.
  • Assist clients in developing, practicing, and applying skills that increase independence, community participation, and meaningful relationships with non-paid community members.

Community Engagement and Collaboration (10%):
  • Build and maintain positive working relationships with clients, community partners, and service providers to support client participation and inclusion in the community.
  • Travel between work sites, attend off-site meetings, and engage with clients and community partners as needed to effectively fulfill job responsibilities.

Accessibility, Equity & Inclusion (5%):
  • Contribute to a respectful and inclusive workplace by demonstrating behaviors aligned with PROVAIL's Equity Statement and Anti-Racist Policy.
  • Collaborate effectively across departments while valuing diverse perspectives and lived experiences.
  • Use inclusive communication practices and support accessibility for colleagues, clients, and visitors.
  • Participate in learning opportunities related to equity, inclusion, and cultural responsiveness.

Additional Responsibilities:
  • Performs related duties as assigned.

What you'll bring:
  • HS Diploma/GED and 1 work year experience
  • In order to fulfill the essential day-to-day travel duties of this position, possession of a current and valid Washington driver's license with a good driving record, access to a vehicle, and maintenance of insurance acceptable to the agency's insurance carrier is required.
  • Must be able to pass a criminal history background check.
  • Lived experience and experience working with individuals with disabilities in a community setting is preferred.
  • Preference will be given to candidates who have familiarity with the community in which the participant resides, including knowledge of community organizations, informal clubs, community projects, and businesses.

WORKING CONDITIONS

This role requires the ability to sit, stand, and/or walk for extended periods, communicate effectively with others verbally & in writing, lift and carry items up to 25 pounds, and operate computer hardware and related systems.
The position requires sufficient visual and hearing abilities to perform job duties safely and effectively, including close and distance vision, color vision, peripheral vision, depth perception, ability to adjust focus, and the ability to hear and respond to verbal communication and environmental sounds. The position also requires frequent standing, walking, bending, and reaching.
