Hourly wage $22.00 - $24.20 ($40,040 - $44,044 a year)

Job Duties:

The successful candidate will be expected to:

  • Work as part of a team to provide case management to individuals receiving services in the community through OPWDD funding (Self Direction program).
  • Support individuals with disabilities to utilize their services and funds to live in the community.
  • Communicate (primarily via phone, email or zoom) with the individuals and their support team, assisting them to utilize their budget according to OPWDD rules and regulations.
  • Obtain and maintain required documentation. Review all documentation for accuracy and guidelines adherence.
  • Document all communications.

Education:

  • A Bachelor or Master of Social Work degree is preferred.
  • A college degree in Human Services or related field will be considered.
  • A high school diploma or equivalent is required.
  • Experience providing OPWDD funded services or other case management services may be considered in lieu of higher education.

Knowledge, Skills & Abilities:

  • 2 years of Case Management experience preferred. Higher education will be considered in lieu of experience.
  • Strong customer service and computer skills.
  • Ability to work in a fast-paced environment.
  • Knowledge of Microsoft Excel, Word, and Outlook.
  • Excellent verbal and written communication skills.
  • Strong organizational skills and attention to detail.
