Job Summary and Qualifications

The RN Case Manager, under the supervision of the Director of Continuum of Care Department provides oversight and intervention through assessing, planning, coordinating, monitoring and evaluation of services required to respond to individual patient needs.

What you will do in this role:

  • Responsible for utilization review, discharge planning and care coordination with the patient, patient’s family, physician and health care team.
  • Works with other members of Continuum of Care Department sharing responsibilities as assigned to meet the needs of each patient.
  • You will plan, implement and evaluate patient care plans before, during and after the procedure to optimize outcomes and maximize available resources.
What qualifications you will need:
  • (RN) Registered Nurse
  • Registered Nurse Diploma
  • Minimum of one year of acute clinical nursing experience preferred.

  • Two years Managed Care and/or Hospital Case Management experience preferred
