Mission-Driven Organization – Cherokee Elder Care is deeply committed to enhancing the lives of elders in our community through a patient‑centered care model. Our goal is to help elders remain safe, independent, and in their homes for as long as possible—avoiding unnecessary placement in nursing facilities. Our participants do not reside at our center; instead, we provide an Adult Day Health Center and serve as their Primary Care Clinic, offering comprehensive support in one convenient location.

· Comprehensive, Interdisciplinary Care Model – As a PACE program, CEC offers a unique opportunity to lead a fully integrated care team that includes primary care, nursing, therapies, social services, transportation, and more—all under one coordinated system.

· Close, Long‑Term Patient Relationships – PACE participants receive ongoing, continuous care, allowing the Medical Director to focus on proactive management, prevention, and meaningful relationships, unlike traditional high‑volume clinical settings.

· Collaborative and Supportive Team Culture – CEC is known for strong teamwork, open communication, and shared commitment to quality care. Medical Directors work closely with an engaged IDT that values physician insight.

Practice Overview & Schedule

· Flexible Scheduling - Preferred in person M-F 8-5 but open to negotiation.

· Balanced Schedule and Predictable Work Environment – PACE programs offer greater stability than traditional medical practice, with minimal on‑call demands. Our nursing team manages all after‑hours and weekend on‑call responsibilities, consulting with the three PCPs only when necessary. The Medical Director is contacted only in rare situations requiring physician‑level input—typically limited to occasional medication decisions or complex guidance. All on‑call responsibilities are handled remotely by phone with documentation, and no staff are ever required to leave their homes while on call.

Patient Population & Scope of Care

· The Chief Medical Director provides overall medical leadership, clinical oversight, regulatory guidance, and interdisciplinary support to ensure high-quality, compliant, and coordinated care for all PACE participants.

· 3 Primary Care Providers have panels consisting of adults aged 55+ managing various chronic health conditions, such as:

o Type 2 Diabetes (DM 2)

o Hypertension (HTN)

o Coronary Artery Disease (CAD)

o Congestive Heart Failure (CHF)

o Kidney Disease, and more.

· We are their insurance: As a Program of All-Inclusive Care for the Elderly (PACE), we operate as a Medicare Part C replacement plan, meaning Cherokee Elder Care serves as the participant’s insurance provider. Any service approved by the IDT and/or Primary Care Provider is fully covered, resulting in no cost to the participant.

Pay and Benefits

· Guaranteed Base Salary + Annual Raise or Bonus

· Sign on Bonus of $15,000

· CME Benefits – $3,500 per year stipend and will not deduct from your PTO bank during CME attendance

· 2 Hours of Mental Health Leave monthly and 8 extra hours of PTO for your birthday
