Position Overview

Change Makers LLC is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote community health team in Paradise Valley, Nevada. Key Responsibilities

• Conduct thorough assessments of clients’ social, emotional, and psychological needs, utilizing a strengths-based approach to identify appropriate interventions.
• Provide individual and group counseling sessions, employing evidence-based therapeutic techniques tailored to each client's unique issues, including trauma, grief, and family dynamics.
• Develop and implement personalized service plans that establish clear goals and outline targeted strategies for client progress, regularly reviewing and adjusting as necessary.
• Advocate passionately for clients to ensure their rights are upheld, facilitating access to essential resources such as housing, healthcare, and educational opportunities.
• Contribute to the evaluation and improvement of existing programs, assisting in grant writing and reporting to secure funding for future initiatives.
• Forge strong partnerships with local organizations, government agencies, and community leaders to enhance service offerings and address the root causes of social injustice.
• Participate in team-focused meetings to share insights, strategies, and success stories, fostering a culture of continuous improvement and support.

Patient Population / Client Focus

In this role, you will primarily support individuals and families from diverse backgrounds who are facing mental health challenges, socioeconomic hardships, and various life transitions. Your work will directly impact marginalized communities, ensuring they have access to effective support systems and the tools necessary for empowerment.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Exceptional communication, empathetic listening, and organizational skills are essential to effectively serve clients and collaborate with team members.
• A strong understanding of social justice issues and trauma-informed care practices is necessary for this role.

Preferred Experience

• Previous experience working within a nonprofit organization or community health setting is preferred.
• Familiarity with case management systems and advocacy program frameworks will enhance your effectiveness in this role.
• Previous involvement in outreach programs will be an advantage in connecting with underserved populations.

Compensation and Benefits
