Case Manager

Job Summary:

As a Case Manager at Behavioral Health Group, you will assist patients in achieving their social, educational, and occupational goals. You will provide referrals, coordinate care, and adhere to all regulatory requirements, reporting to the Counselor Supervisor or Program Director.

Key Responsibilities:

  • Conduct regular drug abuse case management sessions.
  • Orient patients to treatment program operations and procedures.
  • Collect and assess social, educational, and vocational information.
  • Develop and revise patient treatment plans, maintaining comprehensive records.
  • Make appropriate referrals and perform follow-ups.
  • Assist in implementing new programs and advise on caseload management.
  • Provide crisis intervention and inter-agency documentation.
  • Coordinate care with community providers.
  • Achieve annual goals and develop knowledge of addiction and related regulations.
  • Participate in in-service trainings and attend relevant conferences.
  • Adhere to all federal, state, and local regulations and accrediting agency requirements.
  • Participate in community and public relations activities.
  • Commit to diversity and inclusivity in the workplace.

Minimum Requirements:

  • Must have a Bachelors degree in the related Human Services field.
  • Pursuing licensure as a Certified Alcohol and Drug Counselor (CADC) is preferred.
  • Compliance with federal and state certification and licensure regulations.
  • Knowledge of addiction, treatment methods, and current trends in opioid maintenance treatment preferred.

Physical Requirements and Working Conditions:

  • Effective communication and adequate vision.
  • Prolonged sitting, some bending, stooping, or stretching.
  • Standard medical office environment with interactions involving ill or infectious patients, those with mental health diagnoses, or involved in the criminal justice system.
