) Director of Social Services
Job Summary:
We are seeking a compassionate and experienced Director of Social Services to join our dedicated team at AdviniaCare Newton-Wellesley. As a Director of Social Services, you will provide social services to our residents, including admission process, facility adjustment, psychosocial needs, resident grievances, family grievances, MDS assessments, counseling, care plans, discharge planning, and facilitate discharge procedures. If you are passionate about making a positive impact on the lives of others and are looking for a rewarding career, we encourage you to apply.
Responsibilities:
  • Manage the psychosocial, mental, and emotional needs of residents residing in a skilled nursing center
  • Develop and/or aid in the access of services
  • Provide social services in accordance with the National Association of Social Workers (NASW) Code of Ethics and strict adherence to government regulations
  • Work effectively with a variety of disciplines in an individual and team setting
  • Possess a working understanding of social systems and the ability to implement pertinent state and federal regulations
  • Develop and maintain strong relationships with residents, families, and other healthcare professionals

Requirements:
  • Licensed Social Worker: including: LSW, LCSW, LICSW, MSW
  • Skills in communication, assessment, and social work methods and techniques
  • Ability to work effectively in a fast-paced environment
  • Strong organizational and time management skills
  • Ability to maintain confidentiality and handle sensitive information
  • Compassionate and empathetic demeanor with a focus on providing excellent customer service
