Job description

DESCRIPTION
Essential Duties and Responsibilities:
The home health agency Director leads and directs the daily operations of the agency. Coordinates clinical departments and assumes responsibility for continuity, quality, and safety of services delivered in compliance with State and Federal regulations (Conditions of Participation). Understands current Home Health Conditions of Participation and develops and implements clinical changes accordingly. Provides direction to subordinates, in an effort to ensure quality, compliance with physician's Plan of Care and assessment and reassessment of patients' needs and continuity of services by appropriate health care personnel
  • Able to perform OASIS assessments and regular home visits as needed.
  • Directs and evaluates efficiency and professional practice of nursing and HHA staff.
  • Directs and monitors QAPI program and assures appropriate corrective measures are performed.
  • Monitors and identifies problem areas to the Administrator both verbally and through written reports.
  • Promotes and educates regarding the concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
  • Assumes leadership of the case management team.
  • Establish work schedules and assignments for staff, according to workload, space and equipment availability
  • Maintain awareness of advances in nursing, government regulations including development, revision and implementation of department policies and procedures as required
  • Prepare monthly, quarterly, and annual agency reports as required.
  • Measures and monitors patient satisfaction and outcomes.
  • Orientation of new agency personnel.
  • Staff recruitment in collaboration with Human Resources.

REQUIREMENTS
  • BSN with two years registered nursing experience or ADN with 4 years of registered nursing experience
  • Knowledge of Home Health State and Federal guidelines
  • Knowledge of Medicare/Medicaid Home Health Conditions of Participation
  • Knowledge of Medicare/Medicaid Home Health ICD-10 coding, billing and development of 485
  • Current BLS certification
  • Minimum one year Home Health experience
