Director of Nursing (DON)

The Opportunity

We are currently seeking an experienced, compassionate, and highly motivated Director of Nursing (DON) to lead our nursing department in a skilled nursing facility.

The Director of Nursing is responsible for the overall leadership, management, and clinical oversight of nursing services. The ideal candidate will be a strong clinical leader with extensive skilled nursing experience, excellent communication skills, and a thorough understanding of federal and state long-term care regulations.

What You Will Do
  • Provide leadership, direction, and oversight for all nursing services and clinical operations.

  • Manage and supervise nursing staff, including RNs, LPNs, CNAs, nursing supervisors, unit managers, and other clinical team members.

  • Participate in recruitment, hiring, orientation, training, coaching, competency evaluation, and performance management of nursing staff.

  • Ensure appropriate staffing levels and nursing coverage based on resident acuity, census, and regulatory requirements.

  • Develop, implement, and monitor nursing policies, procedures, and standards of practice.

  • Ensure residents receive high-quality, person-centered care in accordance with their individualized plans of care.

  • Monitor changes in resident condition and ensure appropriate physician, family, and interdisciplinary team notification and follow-up.

  • Ensure accurate, timely, and complete clinical documentation and medical records.

  • Collaborate with physicians, nurse practitioners, pharmacy, rehabilitation, dietary, social services, and other interdisciplinary team members.

  • Oversee infection prevention and control practices, antibiotic stewardship, wound care, falls prevention, medication management, and other clinical programs.

  • Monitor quality measures, hospital readmissions, incidents, accidents, grievances, and other clinical performance indicators.

  • Participate in and provide leadership for QAPI, Performance Improvement Plans (PIPs), and facility quality initiatives.

  • Lead survey readiness efforts and assist with Plans of Correction when necessary.

  • Ensure compliance with CMS, State and Federal regulations, HIPAA, OSHA, and all applicable standards of care.

  • Protect and promote resident rights, dignity, privacy, choice, and quality of life.

  • Investigate clinical incidents and concerns and ensure appropriate corrective actions are implemented.

  • Maintain effective communication with residents, families, physicians, staff, and facility leadership.

  • Serve as a member of the facility's leadership team and collaborate with the Administrator on clinical and operational goals.

  • Promote staff accountability, teamwork, professionalism, and a strong culture of resident safety.

  • Perform additional duties as assigned.

What You Must Have
  • Current and unrestricted Registered Nurse (RN) license in the state of practice.

  • Minimum of 5 years of nursing experience in a skilled nursing or long-term care setting.

  • Minimum of 2 years of experience as a Director of Nursing (DON) in a skilled nursing facility.

  • Strong knowledge of CMS and State nursing home regulations and survey requirements.

  • Proven leadership and supervisory experience.

  • Strong understanding of clinical documentation, care planning, quality measures, and medical record requirements.

  • Experience with staffing management, scheduling, and nursing department operations.

  • Ability to effectively lead, coach, and hold nursing staff accountable.

  • Strong clinical assessment and critical-thinking skills.

  • Excellent communication, organizational, and problem-solving abilities.

  • Knowledge of resident rights, abuse prevention, infection control, medication management, and quality assurance requirements.

  • Experience with hospice, acute care, or post-acute care is preferred.

  • Experience with electronic health records in a skilled nursing environment is preferred.
