Our Director of Admission will lead our admission team in ensuring a smooth transition for patients entering our facility, while maintaining our high standards of quality care.

Responsibilities:

  • Lead the admission team in coordinating patient intake, facilitating smooth transitions, and ensuring timely bed availability

  • Collaborate with healthcare providers, families, and patients to develop individualized care plans

  • Conduct thorough assessments to identify patient needs and develop effective admission plans

  • Develop and maintain relationships with healthcare providers and other community partners

  • Prepare and present monthly reports on admission data and trends
