Eden Vista Prospect Heights is a proud Vista Senior Living Management network member, where compassionate care meets exceptional living. With 16 communities spanning across multiple states, Vista Senior Living specializes in Assisted Living and Memory Care services. Our dedicated team ensures a nurturing environment where seniors thrive and find joy in every moment.

Position: Memory Care Coordinator 
Lead activities with residents, engaging them one-on-one and in group settings. Facilitate arts and crafts, music, and physical activities such as exercise and Zumba, while understanding and accommodating residents’ individual interests, abilities, and needs.

Key Responsibilities:

Resident Care & Program Leadership

  • Monitor residents' physical, emotional, behavioral, and cognitive well-being and communicate observations or concerns to the appropriate team members.
  • Ensure compliance with state regulations, dementia care standards, and memory care best practices.
  • Champion person-centered care for residents living with Alzheimer's disease and other dementias.

Memory Care Programming

  • Coordinate and support Memory Care programming designed specifically for individuals living with dementia.
  • Assist in developing engaging, person-centered programming that promotes cognitive engagement, emotional wellness, social interaction, and purposeful daily living.
  • Help tailor activities to each resident's cognitive level, interests, and abilities.
  • Monitor resident participation and provide feedback to help improve programming and enhance residents' quality of life.

Team Support & Collaboration

  • Work closely with the Memory Care, Life Enrichment  and caregiving teams to support consistent, high-quality resident care.
  • Encourage a culture of accountability, compassion, teamwork, and continuous improvement.
  • Communicate effectively with team members regarding resident needs, changes in condition, and care concerns.

Operations & Compliance

  • Maintain accurate documentation, assessments, care plans, programming records, and regulatory compliance.
  • Collaborate with the Executive Director, Memory Care, Life Enrichment team, and other department leaders to support the overall success of the community


What We're Looking For

We are seeking a compassionate Memory Care Coordinator who:

  • Minimum of 2 years experience in Memory Care working exclusively with residents living with Alzheimer's disease and other dementias.
  • Demonstrated experience in a Memory Care community or dedicated Memory Care neighborhood.
  • Strong knowledge of dementia care best practices and state regulations governing Memory Care communities.
  • Excellent communication, organizational, and problem-solving skills.
  • Hands-on approach with the ability to inspire and develop others.
