Service Coordinator Job Summary
We are looking for a highly organized and experienced Service Coordinator in our SL department. You will serve as an advocate for individuals with intellectual and or developmental disabilities to ensure all health and safety needs are met with dignity. You will participate as part of a team to create teaching and support strategies to meet outcomes identified in each individual's plan. Providing mentorship, guidance, and training to our Life Skills Coaches.

Service Coordinator Responsibilities and Duties:

  • Provide staffing coordination to meet state and agency-required coverage to maintain health and safety and manage day-to-day staffing needs for the assigned caseload. Communicate current needs, staffing issues, and all timekeeping management to Human Resources and Payroll.
  • Identify specific needs and wants within the residential home to ensure person centered planning and initiate changes needed.
  • Maintain regular communication with the assigned nurse, Assistant Director, day program coordinator, HR, and individual Interdisciplinary Teams on all pertinent related matters.
  • Active participation in all internal and external audits/surveys
  • Attend ISP and IDT meetings
  • Verify / complete monthly activity schedules
  • Team-driven ongoing emails
  • Monthly updates to annual/semiannual ISP reporting with documentation review
  • Review TSS frequency - work with teams for accuracy and changes as necessary
  • Participate with the team to create TSS and action steps for all individuals on the caseload
  • Ensure nutritional recommendations are met through reviewing mealtime needs for individuals and ensuring coaches/coordinators are aware of needs
  • Teach Therap GER process to coaches and support the ANE reporting process
Requirements
Service Coordinator Job Requirements:
  • Minimum High School Diploma or equivalent
  • Current and Valid Driver's license
  • Safe insured/registered vehicle
  • Must be 25 years of age
  • Completed a minimum of 1-year of human service management or successfully completed the HeartWell Management Training Program.
  • Experience with DD Waiver
  • Work shifts as needed
  • Participate in an on-call rotation

Salary Description
$47,840-$52,000/year
