Job description

Abilene LVN PRN will provide direct patient care to home bound patients.

Responsibilities and Duties:
  • Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP's, PA's, and Physicians.
  • Carries out the clinical plans of care to assist patients maintain their independence.
  • Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
  • Provides education to patients and families on proper home health care procedures and strategies.
  • Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.
  • Qualifications and Skills:
  • Current Texas LVN License.
  • Current Certification in BLS/CPR
  • Valid Texas Driver's License and Automobile Insurance
  • Knowledge of WellSky EMR software is a plus
  • Salary: Pay Per Visit
