Job description

  • Department: Case Management
  • Schedule: Monday-Friday 8am-4:30pm with a weekend rotation every 8-10 weeks
  • Work Location: UAB St. Vincent's Birmingham

Preferred Qualifications:

1 year Case Management experience
1 year Acute Care bedside experience
1 year Managed Care Prior Authorization experience 
1 year Utilization Management/Review experience
CCM, ACM, or ANCC certification


Coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.

  • Creates plan for care across the continuum, integrating patient/family preferences and values.
  • Monitors patient medical necessity and level of care through assessments, ongoing evaluations
    and/or patient records.
  • Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and care transition team members to ensure effective
    implementation and reevaluation of health plan.
  • Acts as a resource for adequate medical record documentation, level of care recommendations, and
    services as they relate to diagnoses, and treatment options for post-discharge care.

Licenses / Certifications / Registrations

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

Education: 

  • Diploma from an accredited school/college of nursing and professional licensure at time of hire required.
