Full-time, 1.0 FTE | Monday-Friday between the hours of 7:00 am-5:00 pm | No routine evening, weekend, or holiday coverage unless otherwise specified

Join a Team Transforming Post-Acute Care Through Telehealth

Oak Medical is seeking an experienced and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing clinical team and help establish an innovative Telehealth Triage Program serving residents in Skilled Nursing Facilities and Assisted Living Facilities.

This unique position combines chronic disease management, routine follow-up care, and same-day urgent clinical triage delivered through telehealth technology. As a key member of our care team, you will collaborate with facility staff, primary care providers, and interdisciplinary teams to improve patient outcomes, reduce unnecessary hospitalizations, and provide timely access to high-quality medical care.

What You'll Do

Chronic Disease Management & Follow-Up Care

  • Conduct scheduled telehealth visits for residents in skilled nursing and assisted living facilities.
  • Evaluate, treat, and manage chronic medical conditions, including:
    • Hypertension
    • Diabetes
    • Heart failure
    • COPD
    • Dementia
    • Depression
    • Chronic pain
  • Review laboratory and diagnostic results and implement treatment plans.
  • Perform medication reconciliation and medication management.
  • Coordinate care transitions and post-hospital follow-up visits.
  • Maintain accurate and timely clinical documentation in the electronic medical record (EMR).

Same-Day Clinical Triage & Urgent Care

  • Respond to urgent clinical requests from facility nursing staff.
  • Evaluate acute changes in condition via telehealth, including:
    • Falls
    • Respiratory symptoms
    • Urinary concerns
    • Altered mental status
    • Skin and wound issues
