• Deliver comprehensive social work services to clients via telehealth platforms, ensuring accessibility and convenience.
• Conduct thorough psychosocial assessments to evaluate clients' needs and develop tailored treatment plans that address emotional, social, and logistical concerns.
• Coordinate with interdisciplinary teams, including physicians, nurses, and chaplains, to ensure holistic care for hospice patients.
• Support clients and families by providing advocacy regarding their rights and access to necessary resources, including community services and support groups.
• Maintain accurate and timely documentation of client interactions, treatment plans, and follow-up actions in compliance with healthcare regulations.
• Engage in regular supervision and professional development, participating in team meetings to share best practices and learn new approaches to caregiver support.

This position primarily focuses on hospice care, involving clients who are experiencing terminal illness and their families coping with grief and stress related to end-of-life issues. • Master's degree in Social Work (MSW) from an accredited university, signifying your educational background and readiness for clinical practice.
• Active Licensed Clinical Social Worker (LCSW) credential in California, demonstrating your professional qualifications and ability to practice independently.
• A minimum of 3 years experience in social work, ideally within hospice, palliative care, or similar settings that require an understanding of complex emotional and social issues.
