Salary $135,000-$140,000 based on Experience and FTE

Oregon Integrated Health is an Integrated Primary Care Medical Home that is hiring for a Full Time Nurse Practitioner for our Clinic in Eugene Oregon. We are a Tier 5 Patient Centered Primary Care Medical Home, Vaccine for Children Program and candidate for Rural Health Clinic designation! We have an amazing team and looking forward to our growth in Eugene!

Our Integrated Medical Home in Portland currently has a team of Family Nurse Practitioners, Naturopathic Physicians, Licensed Professional Counselors, Behavioral Health Clinicians, Chiropractor, Acupuncture, Medical Assistants, Front Desk and community health worker.

We utilize telemedicine visits for patients that are at home as well as telehealth visits – when patients may not have access to data or smart phones patients come to our facility and utilize our technology to connect with any of our providers across our 4 locations! Our additional locations in Florence, Salem and Portland are comprised of Family Nurse Practitioners, Naturopathic Physicians, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage Therapy and oral health with an Expanded Practice Dental Hygienist.

  • $8,000 Sign On Bonus, paid first pay period following 90 days of employment with OIH
  • Salary $135,000-$142,000 based on Experience and FTE
  • Flexible with hours and scheduling
  • Health, Vision, Dental Benefits paid at 50% of Base Plan offered by OIH, eligible after 60 days of employment
  • 401K Benefit: OIH matches 100% of your contributions up to 3% of your compensation, plus OIH matches 50% on your deferrals that are greater than 3%, but less than or equal to 5% of your compensation (Eligibility Requirements: 1yr employment, 1000hrs worked, at least 21yrs of age)
  • Eligible for OIH Benefits after 90 Day Probation Period
  • State Medical License and DEA 100% reimbursed, completing 90 days of employment
  • Continuing Education Benefit up to $700 annually, completing 90 days of employment

  • Protected Sick Time accrues 1 hour for every 30 hours worked, max use of 40 per year, available immediately as time accrues after first day of employment
Job Duties and Responsibilities

· The Primary Care Provider will provide primary care in an integrated setting with various other specialties at the clinic.

· Primary Care Provider will have access to Medical Assistants that will room patients, collect vitals, submit referrals and perform electronic orders as submitted by provider.

· Primary Care Provider will have access to Patient Health Navigators that will follow up on ED visits, referrals, complex care patients and quality metric data.

· New patient visits are 45 minutes and follow up 30 minutes, Urgent appointments are 15 minutes. Same day appointments will be available for each provider.

· Conduct standard medical and physical examinations and perform standard diagnostic tests in office and through telemedicine conferencing.

· Orders, reviews and provides patient instructions regarding diet and activities and their modifications as appropriate.

· Refer patients to other health professionals in the treatment of diseases requiring further diagnosis and evaluation.

· Provides appropriate medical record documentation in an EHR on all patients in accordance with clinic policy.

Qualifications, Skills and Abilities

· Previous experience in an Urgent or Family Care, Emergency preferred

· Comfortable and skilled with electronic platforms such as EHR

· Accredited Medical School Graduate.

· State of Oregon license

· Full prescription authority

· Drug Enforcement Agency Certificate

· National Provider Identifier (NPI)
