Job description

Job Title: Licensed Clinical Social Worker

Department: Clinics

Reports To: CPMO

Direct Reports: None

FLSA Classification: Exempt

Position Summary

This position is responsible for providing as appropriate diagnostic and counseling services to children, adolescents and adults with mental health issues. In collaboration with the multidisciplinary team, performs core functions of psychosocial assessment, care planning, referral and coordination, and counseling/crisis support. Prepares and updates case records, and provides education and support to include available community agencies and resources.

General Duties, Tasks and Responsibilities

  • Provide support and resources for clients; research and refer client to resources.
  • Assess patient needs and develop strategies to help.
  • Completes assessment, diagnosis and treatment planning activities.
  • Assists in educating clients/family about psychotropic medications
  • Respond to crisis situations and provide counseling as needed.
  • Create plan for care across the continuum, integrating patient/family preferences and values.
  • Utilize acceptable clinical techniques to effect client change according to goals/objectives of client.
  • Monitors patient care through assessments, evaluations and/or patient records.
  • Advocates for resources and removal of barriers.
  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
  • Works jointly with other disciplines to arrange service beyond scope.
  • Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.
  • Complies with all established safety procedures to ensure a safe environment for patients, visitors, and staff.
  • Participates in interdisciplinary meetings as needed.
  • Participates in performance improvement activities.
  • Performs other duties as assigned.

Certification/Licensure Requirements

  • IL Licensed Clinical Social Worker (LCSW) required

Experience Requirements

  • 2 years preferred

Computer Skills

  • Strong computer skills including Microsoft Word, Excel and Outlook

Additional Skills

  • Ability to work independently, prioritize and complete tasks in a timely manner
  • Strong communication skills including reading, writing and the ability to listen effectively
  • Capable of taking initiative to make decisions and is an advocate for change and improvement
