Job description

The Nutritionist/Dietitian Provides individualized nutrition counseling and diabetes self-management education/training to individuals and groups. Supports patients in making sustainable lifestyle changes to improve overall health outcomes, with special focus on the diabetic population at Gonzaba Medical Group clinics. Works collaboratively with the clinical team to improve patient care, outcomes, and satisfaction.

This position has no supervisory responsibilities.

All duties performed will be done accurately and in a timely manner.

  1. Provides nutrition assessment, counseling, and individualized meal planning tailored to medical conditions, cultural preferences, and lifestyle.
  2. Offers diabetes self-management education through both group classes and one-on-one sessions.
  3. Collaboratively develops goals, learning objectives, and educational plans with patients.
  4. Documents nutrition and diabetes education encounters, interventions, and outcomes in the medical record.
  5. Develop individualized follow-up plans with patients and monitor progress.
  6. Coordinates with physicians, nurses, and other care team members to support patient goals.
  7. Identifies when patient needs fall outside of scope of practice and arranges for referral to appropriate services.
  8. Develop educational and training materials in collaboration with physicians, dietitians, and diabetes educators.
  9. Participates in HEDIS and other quality improvement efforts to enhance patient care and service.
  10. Conducts staff development and training regarding nutrition and diabetes self-management.
  11. Maintains professional affiliations and enhances professional growth to remain current with best practices in nutrition and diabetes education.
  12. Participates in committee work and projects assigned.
  13. Attends required meetings and contributed to program planning and evaluation.
  14. Maintains strictest confidentiality.
  15. Performs other duties as required.

Education and Training:

Minimum high school education or equivalent. Registered Dietitian (RD/RDN) strongly preferred.  Bachelor’s degree in nutrition, Dietetics, or a related field required. Certified Diabetes Care and Education specialist certification preferred or willing to obtain.

Experience:

Minimum of 2 years of experience in medical nutrition therapy and/or diabetes education is preferred. Experience in chronic disease management, health behavior change, and patient-centered counseling required.

Other Requirements:

Computer Skills: Knowledge of Word processing software, spreadsheet software, Internet, and database software. Skills in using computer/copier. Proficiency in word processing, spreadsheet, and database software. Experience with electronic medical record systems.

Work Environment: 

This role is primarily in an office environment, though there may be periods of time spent in clinic or construction environments depending on projects assigned. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions are common to a clinic environment. Exposure to debris, dust, sharp objects, chemicals and electricity are common to construction environments. Gonzaba Medical Group will provide training and appropriate PPE as required to be successful and safe in your work environments.

Mental / Physical Requirements: 

Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 20 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure.
