US:NV:Carson City OP Nutrition Therapy and Diabetes Education
Part Time
Summary
The Nutrition Counselor and Diabetes Educator provides medical nutrition therapy and diabetes self-management training using appropriate curricula in an outpatient setting. Primary duties include chart review, assessment, nutrition diagnosis, intervention, and evaluation of patient response to intervention. The outpatient diabetes educator/nutrition counselor participates in compliance, quality performance improvement efforts and related committees. This position also plans community activities to support program development and participation, when applicable. The outpatient diabetes educator and nutrition counselor precepts students or dietetic interns, as assigned.
Qualifications
Required
  • Registered Dietitian (RD/RDN) credential that is active and in good standing
  • Nevada-licensed Dietitian
  • Attains the CDCES® credential within the first year of hire
  • Flexibility to orient and work at all Carson Tahoe Health locations
  • Availability to work weekends and holidays as needed

Preferred
  • Experience teaching and supervising dietetic interns or other students

Essential Functions
Nutrition Care and Diabetes Self-Management Training
  • Uses accurate and relevant information and data to identify and prioritize problems relevant to the provided education and/or counseling.
  • Assesses nutrition and relevant clinical status, plans, implements, monitors, and evaluates an individualized plan of care for patients with a variety of nutritional and medical diagnoses in accordance with published standards of practice in nutrition and diabetes care, and standards of professional performance for registered dietitian nutritionists (RDNs).
  • Integrates nutrition and diabetes care with other forms of care being provided to patient.
  • Provides education and counseling for patients and families in an understandable format.
  • Documents care in the patient medical record following department standards of care and timeliness.

Coordination of Nutrition Care
  • Attends and contributes to team meetings, rounds and other care coordination activities as applicable to assigned work areas.
  • Collaborates with other departments and services to plan and implement patient care as necessary in meeting the nutritional and learning needs of the patient.
  • Recognizes nutrition problems that require consultation with or referral to another provider.
  • Participates in discharge planning when applicable.
  • Participates on interdisciplinary team, division, and department committees and projects.

Dietetic Education
  • Develops learning activities for dietetic interns, acts as preceptor, and evaluates dietetic intern performance when assigned.
  • Provides nutrition programs, lectures, and other events to the organization and community as requested or as delegated.

Other
  • Collects and records productivity data as directed.
  • Maintains pertinent patient data necessary for quality assurance/performance improvement.
  • May be reassigned to help with inpatient or other outpatient areas in times of need.
  • Performs other related duties as assigned.
