• Conduct comprehensive biopsychosocial assessments to understand clients’ needs and create personalized treatment plans based on best practices.
• Provide individual, family, and group therapy utilizing evidence-based modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Eye Movement Desensitization and Reprocessing (EMDR).
• Engage in crisis intervention and risk assessments, developing safety plans tailored to each client's situation.
• Collaborate with psychiatrists, psychologists, and other healthcare professionals, ensuring coordinated and effective care delivery.
• Assist clients in navigating available resources, including housing support, financial assistance, and community services, promoting a comprehensive approach to their well-being.
• Maintain complete and accurate clinical documentation in accordance with HIPAA regulations and organizational standards.
• Participate in ongoing case consultations, team meetings, and professional development opportunities to enhance skills and service quality.

The patients you will serve include individuals and families facing terminal illnesses, bereavement, and complex emotional challenges related to end-of-life care. You will be instrumental in fostering emotional resilience, facilitating coping strategies, and providing a compassionate support system during difficult transitions. Your expertise will help clients navigate their feelings and support their loved ones, enhancing the quality of care throughout the hospice experience.

• An active Licensed Clinical Social Worker (LCSW) credential in the state of Oregon is required.
• A Master’s degree in Social Work (MSW) from an accredited institution.
• A minimum of 2 years of direct clinical experience in a mental health or social work setting, preferably with hospice or palliative clients.

• Proficiency in trauma-informed care and familiarity with various therapeutic modalities, including cognitive behavioral therapy and solution-focused approaches.
• Experience in case management and crisis intervention is highly desirable, allowing for immediate support during critical situations.
• Knowledge of electronic health record (EHR) systems to efficiently manage client information is a plus.
• Strong interpersonal, communication, and organizational skills to facilitate effective client interactions and teamwork.
