Job description

The Case Manager assigned to the Personal Supports program will provide person-centered case management services while serving as the primary program resource for staff supporting individuals enrolled in Personal Supports. This role partners closely with Direct Support Professionals, families, and interdisciplinary team members to ensure services are delivered in accordance with each person's goals, applicable regulations, and Providence standards.
This person will pay a key role in promoting service quality, providing guidance and coaching, monitoring documentation and compliance, and supporting consistent, person-centered service delivery across the Personal Supports program.
As a Case Manager, you will be a:
Person-Centered Advocate:
  • Develop, implement, and monitor Person-Centered Plans (PCPs) in collaboration with the person and their support network, which reflect that person's goals and preferences, and prioritize their wellness and needs.
  • Serve as the primary case management contact for individuals receiving Personal Supports services and maintain regular communication with families, caregivers, and interdisciplinary team members.
  • Conduct routine check-ins with people supported, families, and caregivers to assess satisfaction, address concerns, and ensure accountability and continuity of care.
  • Educate Providence staff members about relevant PCPs, and provide ongoing guidance around implementation of identified supports.

Staff Resource & Programs Support:
  • Serve as the primary resource for Direct Support Professionals working within the Personal Supports program by providing guidance, answering questions, and reinforcing person-centered practices.
  • Monitor service implementation and work collaboratively with staff to ensure supports are delivered in accordance with each person's Person-Centered Plan.
  • Identify performance, training, documentation, or service concerns and coach staff appropriately, or take corrective action as determined in partnership with the Director.
  • Support staff understanding of Providence policies, documentation standards, and regulatory requirements.
  • Collaborate with supervisors to promote consistency, quality, and accountability across the Personal Supports program.

Documentation Expert:
  • Monitor documentation within iCare Manager, LTSS, and other required systems to ensure records are accurate, timely, and compliant.
  • Monitor service quality and outcomes to ensure supports remain responsive to changing needs, updating Person-Centered Plans and related documentation as appropriate.
  • Audit documentation and provide coaching and feedback to improve quality, completeness, and regulatory compliance.

Requirements
Case Managers must have:
  • Bachelor's degree or 4 years of similar experience.
  • Previous case management experience preferred.
  • To perform this job successfully, an individual must have knowledge of Microsoft Office Suite Database software (Word, Excel, and Outlook). Experience in Therap, PCIS2 and effective use of agency and state systems, etc.
