Case Manager RN

Provide case management for all medical and surgical patients. Manage patient care in ICU and IMC settings. Collaborate with interdisciplinary teams for patient support. Conduct admission criteria assessments and continued stay reviews. Coordinate discharge planning and facilitate patient transitions. Ensure compliance with CMS and HIPAA guidelines. Utilize charting systems effectively for documentation. Participate in audits and review processes for care management. Support appeals and denials process as needed. Work Monday through Friday with no weekends or holidays required. Maintain a daily patient ratio of 25. Contribute to benefits eligibility and disability case management. Manage medical necessity determinations based on evidence-based guidelines. Assist in care coordination and oversee authorization processes. Implement disease and worker's compensation management protocols. Perform needs assessments and order durable medical equipment when necessary.
