Hospice Social Workers at the DHHC are integral members of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, allowing them to receive the medical care required in the comfort of their own home.

The Hospice Social Worker will:

  1. Provides clinical social evaluations and plans appropriate interventions based on evaluation findings.
  2. Identifies appropriate community resource referrals to address the client’s and the family’s practical and environmental needs.
  3. Acts as an advocate for both the client and the family to navigate the patient through the community system.
  4. Participates in the development of the total plan of care with the multidisciplinary team of clinicians, as appropriate.
  5. Develops and maintains contracts with public and private agencies as resources for the patient.
  6. Educates and instructs patients, family members, or other patient representatives, in community resources to promote the patient's health, safety, well-being and independent living.
  7. Accurately document observations, interventions and evaluations pertaining to patient care management and services.

REQUIRED QUALIFICATIONS

  • Master of Social Work required.
  • Minimum one year experience in a health-related field required.
  • Experience with database software or electronic medical record software required.

PREFERRED QUALIFICATIONS

  • MSW and/or LCSW preferred.
  • Hospice experience preferred.
