Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to patients undergoing detoxification and substance use disorder treatment. Working under the supervision of the Medical Director and/or Registered Nurse, the LPN is responsible for medication administration, patient monitoring, clinical documentation, and coordination of care within a structured, recovery-focused environment.

This position plays a critical role in ensuring safe withdrawal management, medical stability, regulatory compliance, and compassionate patient-centered care.

Essential Duties & Responsibilities

Medication Management & Clinical Oversight

  • Administer prescribed medications, including oral, IM, and injectable medications, in accordance with physician orders and nursing scope of practice.
  • Manage medication room operations, including inventory control, storage compliance, narcotic counts, and medication reconciliation.
  • Process medication orders, prior authorizations, and coordinate with pharmacy vendors.
  • Monitor patients for medication effectiveness, side effects, and adverse reactions.
  • Document all medication administration accurately in the EMR.

Patient Assessment & Monitoring

  • Conduct nursing assessments upon admission, during treatment, and at discharge.
  • Monitor and record vital signs, withdrawal symptoms, and behavioral observations.
  • Assess and report changes in patient condition promptly to the RN, provider, or Medical Director.
  • Assist in the development and modification of individualized treatment plans/care plans.
  • Monitor diet, hydration, physical activity, and overall wellness.
  • Conduct nursing education groups and document accordingly in patients' chart.

Admissions & Discharge Coordination

  • Complete intake nursing evaluations and process admissions in collaboration with clinical and medical teams.
  • Ensure proper documentation for patient transfers and discharge instructions.
  • Provide patient education regarding medications, detox expectations, and aftercare planning.

Laboratory & Diagnostic Support

  • Conduct and document specified laboratory tests (e.g., urine drug screens, blood glucose monitoring).
  • Assist in ordering, collecting, and documenting diagnostic tests as directed.
  • Interpret basic clinical data and report findings appropriately.

Crisis Response & Reporting

  • Identify and respond to medical or psychiatric crisis situations.
  • Immediately report incidents of abuse, neglect, or misappropriation per policy.
  • Notify the Clinical Director and/or Medical Director of emergent concerns.
  • Follow facility protocols for incident reporting and emergency response.
  • Support incident investigations and reporting requirements.

Compliance & Collaboration

  • Ensure nursing practices align with state regulations, accreditation standards, and facility policies.
  • Work closely with clinical, medical, and operations teams to maintain compliance readiness.
  • Participate in multidisciplinary treatment team meetings.
  • Maintain strict HIPAA compliance and patient confidentiality.
  • Ensure required patient care documentation is completed according to policy.

Professional Development

  • Participate in ongoing education, required certifications, and staff development activities.
  • Assist in training new nursing staff or support personnel as needed.
  • Contribute to quality improvement initiatives and systems evaluation efforts.

Minimum Qualifications (Knowledge, Skills, & Abilities)

  • Active and unrestricted Licensed Practical Nurse (LPN) license
  • Prior experience in addiction treatment, behavioral health, detox, or acute care preferred
  • Knowledge of withdrawal management protocols and medication-assisted treatment (MAT) preferred
  • Current CPR/BLS certification (or ability to obtain upon hire)
  • Strong documentation and EMR proficiency
