Summary

This position obtains and processes the necessary information for the health system's vital statistics requirements specific to birth certificates and death certificates. Information is obtained and processed for: the completion of birth certificates as required by the State of North Carolina, for the completion of death certificates and death related paperwork, and for coordinating and supporting functions with the Medical Examiner, with funeral home representatives and with the vital statistics division of the county and state public health departments. This position serves as the liaison between the Health System, funeral home establishments, the County and State Public Health Departments and other areas as neededMajor Job Functions

The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below. In addition, specific functions may change from time to time:

  • Prepares birth certificates by retrieving Birth Delivery Notification sheets, reports from Labor/Delivery, recording infants' names in birth log, interviewing mothers to obtain information, inputs the birth certificate information, obtains signatures as required

  • Uploads birth information to the State Vital Records Office, Raleigh, NC on a regular basis.

  • Obtains information and prepares paperwork for deaths to include in-hospital deaths, stillbirths, medical examiner related cases, deaths occurring in the Emergency Department and DOAs.

  • Supports the Medical Examiner by preparing the Medical Examiner's death certificate and other essential paperwork

  • Assists bereaved families with information about releasing the deceased to a funeral home and by explaining paperwork to the families

  • Coordinates release of bodies with funeral directors and Health System staff

  • Reports and investigates problems, initiates appropriate corrective action, and advises the direct supervisor, department director or other available member of the HIM Management Team of any emergencies or matters which warrant further attention

  • Provides notary services for patients and their families as part of the birth certificate process

  • Practices Erickson's developmental model and demonstrates age specific interventions

  • Is knowledgeable of and sensitive to the psychosocial, spiritual and cultural needs of individuals

  • Demonstrates workload management

  • Other duties as assigned

Minimum Qualifications

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:

Education and Formal Training:
  • Associate degree in Health Information Management or related program, or equivalent in work experience, education, or training

  • RHIT, RHIA preferred

  • Must obtain a notary public appointment within 30 days of completion of probationary period

Work Experience:
  • Two years' acute care HIM or vital statistics experience preferred

  • Minimum of two years' experience in customer service or working with the public

  • Public/guest relations experience desirable

Knowledge, Skills, and Abilities Required:
  • Proficiency in reading, writing, and speaking the English language

  • Ability to use word processing programs including Microsoft Windows

  • Type accurately 50 words per minute

  • Good organizational skills

  • Good telephone etiquette

  • Ability to work with a variety of people in difficult situations

Physical Requirements:
  • Ability to speak and hear clearly

  • Visual acuity and motor coordination and manual dexterity

  • Must be able to walk to various departments of the Health System
