Job description

Nursing Home Physician

New York City – Multiple Boroughs (Bronx, Brooklyn, Poughkeepsie, Westchester County)

Position Overview

A large multispecialty group is seeking Board Certified or Board Eligible Physicians to join our Nursing Home Division. Our expanding primary care team provides high-quality diagnostic and treatment services to patients across NYC and surrounding areas.

Physicians in this role will meet, assess, and diagnose patients in nursing homes, providing comprehensive care that adheres to state and national standards. The ideal candidate is passionate about primary care, diagnostics, and collaborative teamwork, and enjoys working closely with other healthcare professionals to improve patient outcomes.

Key Responsibilities

Patient Care & Evaluation

  • Provide primary medical care to nursing home patients within standard of care guidelines

  • Conduct assessments, review medical histories, and analyze laboratory and imaging results

  • Diagnose medical conditions and develop individualized treatment plans

  • Prescribe medications and treatment regimens, considering allergies and potential side effects

  • Provide lifestyle and diet guidance to improve patient health and prevent disease

Collaboration & Coordination

  • Work closely with nursing home staff, specialists, and the primary healthcare team

  • Refer patients to appropriate specialists when necessary

  • Participate in quality improvement initiatives, safety standards, and infection control programs

Documentation & Compliance

  • Maintain complete, accurate, and timely medical records in eClinicalWorks

  • Ensure compliance with HIPAA, state, and federal privacy regulations

Qualifications
  • Board Certified / Board Eligible in Geriatrics, Internal Medicine, Palliative Care/Hospice, or Family Medicine

  • MD or DO degree from an accredited U.S. medical school

  • Completion of an accredited residency program or U.S. residency / 5-year pathway

  • Active New York State Medical License

  • DEA and DPS registration (may be obtained during credentialing)

  • Clinical experience providing primary care and diagnostics in a team-based setting

  • Experience with EMR systems (eClinicalWorks preferred)

  • Strong interpersonal, communication, and clinical judgment skills
