Licensed Practical Nurse (LPN) – Day Shift
Silver Stream Rehabilitation and Nursing Center
7:00 AM – 7:00 PM | 3 Days Per Week | Every Other Weekend

Silver Stream Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. This position offers a consistent schedule of three 12-hour day shifts per week (7:00 AM–7:00 PM), including every other weekend, providing an excellent work-life balance while making a meaningful difference in the lives of our residents.

Responsibilities:

  • Administer medications and treatments as prescribed

  • Monitor residents' conditions and report changes to the RN Supervisor and physicians

  • Provide direct nursing care in accordance with facility policies and standards of practice

  • Accurately document resident care and services provided

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Participate in care planning and interdisciplinary team meetings

  • Ensure compliance with all state, federal, and facility regulations

Qualifications:

  • Current LPN license in the Commonwealth of Pennsylvania

  • Long-term care experience preferred, but not required

  • Strong clinical, communication, and organizational skills

  • Ability to work effectively as part of a team

  • Commitment to providing exceptional resident-centered care
