WHAT: Visit Nurse Home Health WHERE: Port Angeles and Sequim STATUS: Full Time/1.0 - 40 hours/week - 5x8s SHIFT: Day Shift, Weekends/Holidays TIMEFRAME: 3 Month Temporary Assignment The Visit RN helps provide continuity of care by providing nursing visits for care of patients as delegated by the Case Manager RN who remains responsible for the overall planning and coordination of care for a caseload of patients. The Visit RN member of the interdisciplinary team provides nursing care to the patient in the patient's place of residence as prescribed by the physician and in compliance with applicable laws, regulations and policies of the agency. Keeps the Case Manager RN informed about patient progress, recommended actions and physician orders, and plans for the next visits. Individuals assigned to this classification are expected to apply full working knowledge of standards of nursing principles, practices, and to demonstrate problem-solving skills and make independent decisions in complex and diversified work situations under general supervision. Education Associate's degree required. BSN preferred. Experience Minimum of one year recent experience in home health, acute or long-term care. Licensure/Credentials Current Washington State Registered Nurse license. Current BLS certification; valid Washington driver's license; proof of insurance on automobile used for work; reliable automobile transportation to travel to patient residences; driving record in compliance with OMC standards currently in effect or as may be amended.
