Job Description

Program Manager will develop a foundation for identifying and intervening to improve the health of rising-risk patients who live work or go to school within the El Camino Healthcare District. Initiatives will focus primarily on the Medicare population and on patients with commercial insurance who fall into the missing middle.

Job Description

  • Design, develop, and lead the measurement and evaluation strategy for El Camino Healthcare District's Population Health initiatives. Key responsibilities include managing a metrics framework to track and evaluate program activation and outcomes; defining and building key data gathering and infrastructure to support evaluation.
  • Lead program and workstream design and development for future Population Health workstreams, using a data and research-based approach to assess best practices, conduct primary research and data gathering through interviews and research with local stakeholders, and define a comprehensive approach to design and delivery of future in-person and virtual programming.
  • Develop standard population health workflows to support participant engagement, initial enrollment, and sustained participation with ECHD's Population Health programming.
  • Provide subject matter expertise across primary care and public health, to effectively address the local health needs of the El Camino Healthcare District's population and to improve outcomes.
  • Using data available in Epic and/or other sources, identify the target populations that could most benefit from population health intervention.
  • In collaboration with Quality, identify additional data which may need to be collected in Epic and/or other sources in order to track potential health disparities which may inform the population health interventions.
  • Identify champion physician(s) and collaborating departments within ECH.
  • Monitor, track progress, and evaluate progress against the ECHD population health strategy over the next 1-3 years.
  • Support community outreach & engagement execution and strategy development.

Qualifications

  • Bachelor's degree in Public Health, Health Administration, Social Work or a related field required. An advanced Masters' or higher terminal (e.g., PhD) degree in Public Health, Health Administration, Social Work or a related field is preferred
  • Must have at least -5 years' experience managing increasingly complex projects in a healthcare environment
  • Demonstrated knowledge of population health management concepts and strategies
  • Experience using existing literature to drive fundamental program design based in research and existing models, and comfort proposing logical adaptations for ECHD-specific needs
  • Ability to quickly and thoroughly review and evaluate existing bodies of research, synthesize findings, and support decision making in support of key workstreams, population-level analysis, and future strategy definition
  • Experience and ability to operationalize and execute a recurring data capture and reporting cadence; comfort with pulling data from multiple disparate sources and owners; and comfort maintaining a simple but flexible and functional database for tracking core program data and results
  • Experience creating dashboards, crafting narratives and storytelling results from the data, and presenting key reports to key executive stakeholders
  • Ability to work effectively and efficiently toward goals in a complex, diverse environment with multiple and changing demands
  • Must have an outstanding grasp of information technology concepts and processes
  • Demonstrated communication effectiveness and good project management discipline
  • Strong data analysis, research and program evaluation skills
  • Ability to foster trusting relationships with colleagues and clients

Salary Range:

$66.85 - $100.27 USD Hour
