Intake RN

Responsibilities
The Intake RN works closely with the Director of Nursing (DON) and nursing team to coordinate patient admissions, perform comprehensive clinical assessments, and ensure a safe, high-quality care environment. This role combines advanced nursing expertise with patient-focused coordination to support effective intake processes and continuity of care.

  • Conduct thorough nursing assessments to determine patient appropriateness for admission

  • Oversee and manage patient admissions, transfers, and discharges

  • Collaborate with nursing staff to monitor, assess, and document patient care during intake

  • Communicate treatment plans and coordinate with providers to ensure optimal patient care

  • Identify areas of non-compliance and provide in-services as needed

  • Complete intake-related audits and ensure daily checklists are accurate and completed on time

  • Provide detailed patient handoffs to nursing staff at shift changes

  • Identify high-risk patients and communicate with providers regarding care plans

  • Ensure proper documentation and follow procedures for patients requiring higher levels of care (HLOC)

  • Coordinate return to facility for patients sent to HLOC as necessary

  • Administer medications safely and efficiently, ensuring accurate documentation

  • Conduct random narcotic counts and follow medication handling protocols

  • Monitor medication availability and follow up with pharmacy as needed

  • Assist in conducting weekly urine drug screens

  • Receive, analyze, and upload laboratory results, communicating findings to providers

  • Complete administrative duties as assigned by the DON

  • Perform additional tasks as needed to support patient care and intake operations

Qualifications

  • Active, unencumbered RN license in the state of practice

  • Valid BLS certification required

  • Associate?s degree in Nursing or higher required

  • Minimum 2 years of nursing experience in a behavioral health, mental health, or substance use treatment setting preferred

  • Experience in chemical dependency/addiction treatment preferred

  • Personal passion or interest in substance use recovery strongly preferred

  • Commitment to a drug-free lifestyle and responsible alcohol use

  • Leaders in active recovery must possess a minimum of 2 years of sobriety
