Job Title: Program Nurse - LPNDepartment: Adult Day ProgramSupervisor: Program DirectorPart Time: 16 to 22.5 hours per week (2 to 3 days per week)  Job Objective: Coordinates delivery of nursing and social services for adult day participants Minimum QualificationsEducation: Graduate of accredited nursing schoolExperience: 2-3 years of nursing experience. Experience working with the elderly and those with dementia/Alzheimer's Disease, preferred work in a community-based setting and knowledge of services and entitlements for frail elderly are a plus.Licensure: State of Connecticut Nursing License; CPR certification Job Standards1. Demonstrates clear, compassionate, and effective nursing, demonstrating excellent judgement.2. Communicates effectively with clients, employees, and care partners.3. Ability to maintain positive working relationships both within the organization and community.4. Empathetic, culturally aware, inclusive, and person-centered of all we serve.5.
