Horizon House LPN Position

Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual/disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified Licensed Practical Nurse (LPN) interested in joining our Residential Group Home team at our Old Baltimore Pike, Newark, Delaware location.

Part-Time Work Schedule:

  • Days: 2-day business work week
  • Hours: 3:00 PM โ€“ 11:00 PM

Hiring requirements include: completion of new hire orientation and training, first four days and workweek (Mondayโ€“Thursday, day shift) at our Philadelphia location. This also includes eligibility for mileage reimbursement.

Some of the responsibilities include the following.

  • Assist in the development, implementation, evaluation and revision of recovery plans in collaboration with the participants and assigned staff.
  • Maintains a case load of assigned participants and completes assigned paperwork in a timely manner.
  • Act as a liaison consulting with community agencies and families to maintain coordination of care in the treatment process.
  • Provides ongoing assessment of participant's mental health symptoms and responds to recovery plans.
  • Educates and supervises participants in self administration of medication.
  • Ensures annual review of the participants physical condition and assist access to and encourage compliance with treatment for identified physical problems.

Qualifications:

  • Board Certified Licensed LPN in the state of Delaware; or participating in the Nursing Licensure Compact to include the state of DE.
  • Knowledge of mental health systems and supports.
  • Knowledge of substance abuse systems and supports.
  • Good communication skills both oral and written.
  • Ability to work independently.
  • Possess a valid driver's license with an acceptable driving record.
  • Strong organizational skills.

Scope:

  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, stack, stoop, reach and bend which may include direct intervention with participants.
  • Local travel required, including transporting participants.
  • May be required to report to work during emergencies, including inclement weather.
  • May require crisis intervention in emergency situations.
  • Scheduling flexibility and possible on-call work are necessary.
