Job description

Responsible for coordinating the daily activities of HIV Prevention grant-funded programs. The Program Manager will identify site-specific clinical champions and facilitate ongoing clinic staff education and training, establishing and maintaining cross-site collaborations and recruiting and engaging clients in the PrEP programs at each site. Responsible for client tracking, data collection and entry supervision, and service delivery. Coordinates client follow-up and case tracking for patients linking to HIV prevention services at WHMC and for patients testing HIV positive at WHMC or presenting to WHMC to link to HIV care. Participates in Quality Assurance and Evaluation activities, trainings, supervision, and other related activities.
  • Ensure that issues/concerns encountered or raised at/by the patients are resolved; to ensure the smooth operation of the program, including troubleshooting any crisis with patients and/or the care team.
  • Provides direct prevention services to patients as needed
  • Participate and aid the development process for outreach services and online media
  • Attend and facilitate community, program, and staff meetings and training
  • Address client/provider concerns before escalating up to the Director
  • Manage and coordinate the daily activities of the PrEP program at WHMC, including client recruitment, enrollment, intake, assessment, education, follow-up, and cross-site collaborations and referrals to ensure client flow
  • Track and monitor all patients who enroll in the designated grant-funded program(s). Verify that all patients receive appropriate health visits and screenings, test results, linkage HIV/STIs/HCV treatment, timely access to medication, and patient financial assistance.
  • Ensure that all program activities are documented within 24-48 hours comprehensive fashion, including client charts, program databases, linkages with the contracted agency, and outreach activities logs.
  • Submit monthly reports as requested by the Director of Prevention on time.
  • Facilitate regular team meetings and conduct individual supervision with program staff to ensure regular program evaluation, planning, and communication of program objectives and progress.
  • Facilitate clinical staff training and education, PrEP protocol design and implementation, and program marketing
  • Provide backup coverage to program staff as needed in the event of staff turnover.
  • Participate in regular supervision with the Director of Prevention and attend staff meetings as required
  • Attend necessary in-services and meetings off-site with the NYC and NYS Department of Health and AIDS Institute partners to remain abreast of current best practices in HIV prevention
  • Attend designated training, train staff on measures and outcomes, and conducts presentations as requested
  • Develop and maintain active collaborations with community agencies and consumer venues, community boards, and Department of Health initiatives to facilitate outreach, messaging/education, and referrals for clients served by each program
  • Attend meetings after standard working hours or change shifts to facilitate program recruitment and service delivery goals.
  • Identify, conduct, and coordinate quality improvement (QI) activities for the PrEP program and participate in regular QI activities with Senior Administrators/Directors and Grant Managers.
  • Meet with Data Manager after program reports are generated to review accuracy and correct errors.
  • Manage input of data into eShare and AIRS and monitor all patient outcomes
  • Ensures all PrEP outcomes, evaluations, reports, documentation, and other programmatic items are submitted by their deadlines
  • Ensure that staff is working effectively with clients and meeting their treatment goals
  • Monitoring of referral list clients to ensure that mental health services are provided on time
  • Must be able to meet productivity requirements, handle a large workload, work cohesively with other team members, adhere to deadlines and quality assurance reviews, and follow the procedures and protocols of the Department and WHMC
  • Represent the organization at community forums and provide PrEP program information
  • Responsible for program oversight to maintain up-to-date records and create systems to collect and report the data efficiently and accurately
  • Perform other duties as assigned.

Position Requirements:
  • This position requires a minimum formal education of a Bachelor's Degree and a minimum of 3 years of job-related experience.
  • 2-5 years of experience in HIV Prevention and Project Coordination required
  • Strong written and verbal communication skills
  • Computer Literacy in Microsoft Suite required
  • Ability to work as a member of an interdisciplinary team
  • Good problem-solving and conflict-resolution skills
  • Must have excellent client engagement, time management, and record-keeping skills
  • Excellent command of the English language, especially the rules of syntax, punctuation, and grammar

Desired (not required) criteria include:
  • Preferred educational degrees: Masters in Public Health or Community Health. Experience providing HIV/STI testing or treatment and implementing projects with youth and LGBTQ networks.
  • Experience using electronic medical records and electronic databases
  • Certification in HIV testing and counseling
  • Bilingual (English/Spanish)
