Job Summary
The Data Integrity Specialist, Enterprise Master Patient Index (EMPI) is responsible for ensuring the accuracy, consistency, and reliability of patient information within the Electronic Health Record (EHR) and Enterprise Master Patient Index (EMPI) systems. This role reviews and corrects reported data errors with the available chart correction tools. The specialist ensures high-quality data to promote patient safety and seamless care delivery.Job Description
Minimum Qualifications
Education: Associate's degree in Health Information Management, Nursing, Information Technology or healthcare-related field required. In lieu of a degree, two (2) additional years of experience in EMPI corrections and data integrity required.
Licenses/Certifications: One of the following is preferred: Registered Health Information Administrator (RHIA) or Registered Health Information Technician (RHIT).
Experience / Knowledge / Skills:
  • Two (2) years of experience in health information management or in a healthcare administrative, registration, information services, or other healthcare business role required.
  • Experience in chart review with an emphasis on data integrity.
  • Experience working with medical record applications (EPIC preferred).
  • Excellent computer skills, with knowledge of Microsoft Office programs (Excel, Word, Outlook, PowerPoint); HL7 interface message comprehension preferred.
  • Effective oral and written communication skills.
  • Demonstrates critical thinking, strong problem solving, attention to detail, and ability to work independently while collaborating effectively within team environments.

Principal Accountabilities
  • Reviews the medical record and investigates all clinical and non-clinical data, as well as technical interface details to complete the critical EMPI correction process. Completes remediation tasks regarding medical record merges, unmerges, patient data corrections, and medical record overlay corrections.
  • Works daily work queues and escalates complex identity issues.
  • Works in various applications to identify and correct duplicate medical record numbers, overlays, and documentation data integrity issues.
  • Responds to service tickets, prioritizes them, and processes the incoming requests for medical record corrections in a timely manner and in consideration of patient care and safety.
  • Responds to questions received via emails, phone calls, and/or information service tickets regarding chart corrections.
  • Communicates with members of the data integrity team, management team and other end users when needed for resolution of EMPI issues.
  • Reviews, validates and merges duplicate medical records.
  • Performs other EMPI remediation tasks as assigned including matching for payer rosters, miscellaneous data loads, or linking and relinking incoming messages
  • Provides follow-up information on duplicates and overlays regarding status of requests and assignments.
  • Makes document adjustments at both patient and encounter levels via corrections process and re-indexes documents when appropriate.
  • Maintains and updates EMPI correction log as appropriate.
  • Maintains an effective level of coordination on all assigned projects.
  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
  • Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences. Models Memorial Hermann's service standards by providing safe, caring, personalized and efficient experiences to patients and colleagues.
