Southridge Behavioral Hospital is a beautiful and new state-of-the-art facility. As a joint venture with the Trinity Health System and Universal Health Services, we serve up to 96 adult and geriatric patients across four units.

Position Description:
The Intake Clinician, under the supervision of the Intake Director, provides telephonic and in-person risk assessment to assist the physician in identification of appropriate level of care for potential patients. The Intake Clinician is the initial contact with referral sources and potential patients, coordinates the referral and intake process, facilitates the admission process and maintains communication with referral sources, families, patients, and unit staff. The Intake Clinician works with the Business Office regarding patient financial data and verification and prior authorization of insurance benefits as needed, as well as with the UR Department for the certification of patients admitted.
Hours and Location:
Flexible Per Diem Schedule. 7pm to 7:30am.
We are located on the south side of Grand Rapids, near the intersection of M-6 and Byron Center Ave SW: 6291 Venture Hills Boulevard SW, Byron Center, MI 49315
Qualifications
Job Requirements:
Education: Masters Degree required.
Experience: A minimum of three years' experience in a psychiatric health-care facility preferred.
License: Active MA Level License in the state of Michigan: LMSW, LLMSW, LLPC, LPC, LLP, TLLP
