Job Title: Heart Transplant Coordinator

Description:
Coordinates all aspects of care for transplant patients, including inpatient and outpatient services.
Conducts record reviews, medication refills, and prior authorizations.
Provides patient education and reviews test, procedure, and lab results.
Makes referrals for medically necessary services, consults, equipment, and supplies.
Acts as a liaison between administrative, clinical staff, and physicians.
Assists with patient discharge planning as necessary.
Provides education regarding care and medications to patients.
Carries out patient care orders as directed by physicians and in compliance with hospital policy.
Assists with patient admissions and discharges as needed.
Participates in educational programs about transplantation for hospitals, medical specialty units, patient groups, and the community.
Responsible for pre and post-transplant patient medical management, assessment, and discharge planning.
Experience with transplant populations is required.
