Position Summary :

The Regional Navigator Case Manager is the member of the care team responsible for initiating and maintaining effective working relationships with youth, families, referral sources and community resources--prior to admission, through placement, and after transition/discharge. This position will support the Regional Navigator Program for youth and young adult survivors of human trafficking. The Regional Navigator Case Manager acts as a single point of contact to ensure effective communication and organized service provision. The Regional Navigator Case Manager is also responsible for the development and review of a comprehensive needs assessment and ongoing, frequent safety planning with the youth. The Regional Navigator Case Manager works to promote and identify resources available for the youth and family in their home community to build natural supports that will enhance their ability to increase self-sufficiency. An essential responsibility of the Regional Navigator Case Manager is family engagement and identification of strengths and needs. The Regional Navigator Case Manager will collaborate with community partners to encourage consistent screening and referrals of suspected survivors of child sex trafficking, advocate for youth needs, identify resources, form working relationships, conduct warm handoffs, and promote public awareness through community education and advocacy.


Required Education and Experience

  • Bachelor's Degree and at least one year of experience providing direct services to children/youth and families; or
    High School Diploma and a minimum of three (3) years of experience providing direct services to children/youth and families.
  • One year of experience in any of the following:
    1. Working with survivors of trafficking, sexual assault, domestic or intimate partner violence, or other crimes or trauma.
    2. Working in child and young adult systems of care and/or multi-disciplinary teams, such as DSS, CPS, DJS, Child Advocacy Center, law enforcement, or State's Attorney's Office.
    3. Working in any emergency or crisis response services, such as mobile crisis, rape crisis or suicide prevention hotlines, EMT, or emergency departments.


What does the Regional Navigator Case Manager do specifically?

    Essential Functions:

    • Model and act in accordance with EVERSTAND's core values: safety, empathy, relationships, and impact.
    • Demonstrate and practice EVERSTAND's leadership mindset, "Choose kindness. Solve problems and learn together. Make it happen."
    • Collaborate with first responders and other community partners to swiftly deliver emergency assistance and essential supplies to designated youth promptly.
      Complete designated admission service needs assessments that include participation by child/youth, parents/guardians in the identification of strengths, service needs, and transition/discharge planning.
    • Complete ongoing functional assessments at designated timeframes that include participation by child/youth, parents/guardians and appropriate external stakeholders.
    • Participate in regular care coordination meetings, including FTDMs, as well as court appointments, school meetings, community providers meetings, group, individual and family meetings as scheduled.
    • Participate as an active member of a care team for each youth on an assigned caseload. Understanding that the care team includes members who are employed at EVERSTAND as well as the youth's family, external health providers, church community, and any support that the youth and family identify.
    • Promote an understanding of available EVERSTAND and community resources.
    • Promote awareness and education about human trafficking and RNP services and initiatives through training, conferences, and community events.
    • Locate and facilitate referrals to community-based resources within the youth's home community. Coordinate with the EVERSTAND staff members and community resources to address clients' service needs. Ensure that the service needs are meet.
    • Evaluate the child's/youth's/family's progress towards goal attainment, in line with scheduled treatment planning and/or care coordination meetings.
    • Maintain effective working relationships with families, children/youth, colleagues, consultants and community agencies to include scheduling home visits, notifications of serious incidents, follow-up on all ongoing documentation needs.
    • Actively participate in transition and discharge planning for the youth as needed to ensure ongoing support of the youth and smooth transition to identified post-discharge services.
    • Ensure post-discharge follow-up-30 days, 60 days, and 90 days after discharge-to collect data that accurately reflects the residual impact of treatment.


    Position Type/Expected Hours of Work:

    This is a full-time position. Days and hours of work are typically Monday through Friday, 8:30 a.m. to 5 p.m. Candidates must be able to work flexible hours - including evening, overnight, weekends and holidays. This position regularly requires long hours and weekend work due to the nature of the population served.


    Travel

    Travel is required throughout Anne Arundel County service area. Some out-of-area, weekend, and overnight travel may be expected. Also, transportation of clients to community-based meetings and activities to include court hearings, victim/legal advocacy and other activities to support community integration will be expected. May be eligible for mileage reimbursement, contingent upon budget availability within the designated grant period
