As a Home Health Physical Therapist, you'll provide personalized, one-on-one care that helps patients regain strength, improve mobility, restore balance, and rebuild confidence-all in the comfort of their own homes.

You'll have the freedom to create individualized treatment plans, build meaningful relationships, and see the direct impact of your care every single day.

What You'll Do
  • Complete physical therapy evaluations, Start of Care (SOC), reassessments, and discharge visits
  • Develop individualized plans of care based on patient goals and clinical needs
  • Provide skilled physical therapy services in patients' homes
  • Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
  • Monitor patient progress and modify treatment plans as appropriate
  • Collaborate with physicians, nurses, and the interdisciplinary care team
  • Complete timely and accurate clinical documentation using WellSky
Qualifications
  • Active Virginia Physical Therapist License Required
  • Valid Driver's License and Reliable Transportation
  • Strong Clinical Assessment, Communication, and Documentation Skills
  • Home Health Experience Preferred but Not Required
  • OASIS Experience Preferred
  • Experience with WellSky Home Health is a plus
  • Passion for Delivering Exceptional Patient-Centered Care
