Human Touch Home Health is seeking a PRN Physical Therapist (PT) to serve patients along the I-95 Corridor from Triangle to Springfield, Virginia.

Patient opportunities may include Triangle, Dumfries, Quantico, Woodbridge, Dale City, Lake Ridge, Lorton, Newington, Springfield, and surrounding communities.

You do not have to cover the entire corridor. Choose the areas that work best for you and build your caseload around your availability.

What You'll Do
  • Complete physical therapy evaluations in patients' homes
  • Develop individualized plans of care
  • Provide therapeutic exercise, gait, balance, transfer, and mobility training
  • Help patients improve strength, endurance, and functional independence
  • Assess home safety and identify fall risks
  • Educate patients and caregivers on mobility, safety, and home exercise programs
  • Monitor patient progress and adjust treatment plans as needed
  • Complete reassessments and discharge planning
  • Coordinate care with physicians, nurses, therapists, and other team members
  • Complete accurate and timely documentation using WellSky
What We're Looking For
  • Active Virginia Physical Therapist License Required
  • Graduate of an accredited Physical Therapy program
  • Valid Driver's License and reliable transportation
  • Strong clinical assessment and communication skills
  • Ability to work independently in patients' homes
  • Home Health Experience Preferred, but Not Required
  • WellSky Experience is a Plus
