Join Stratford VNA as a Full-Time, Part-Time or Per Diem RN Admissions Nurse in Home Care and enjoy the autonomy of home health nursing while making a meaningful difference in the lives of patients and families throughout the community. In this role, you will provide Assess and Evaluate patients in their homes for admissions to home health care services. You will create and develop individualized care plans, coordinate with physicians and interdisciplinary team members, educate patients and families, and help individuals achieve greater independence and improved health outcomes. Full-Time, Part-Time and Per diem Admission Nurse positions available.
