Salus Hospice is urgently seeking a compassionate and skilled Hospice RN Case Manager/Admissions to join our dedicated team serving the Riverside County region. If you're passionate about providing exceptional hospice care and want to work in a collaborative, supportive environment, this is the opportunity you've been waiting for!
Areas of coverage include: Hemet, San Jacinto, Lake Elsinore, Corona, Murrieta, and surrounding areas
Full Time and Per Diem openings!!
Hospice RN Case Manager Responsibilities:
* Perform RN duties on hospice team.
* Serve as a RN Case Manager for the hospice Interdisciplinary Team
* Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
* Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate
* Coordinates all hospice services provided to patients/families in all settings to ensure quality of care
Ideal Hospice RN Case Manager Candidate Background:
* Registered Nurse with valid CA state license
* Minimum one year of hospice RN experience
* Strong background in medical/surgical, ICU/CCU, or ER is helpful
* Valid driver's license, auto insurance, reliable transportation and willingness to drive to patients place of residence
* Must have excellent interpersonal skills and a passion for hospice
* Understanding of hospice philosophy and needs of the terminally ill
* Self-directed with ability to work with little supervision
* Current Physical and TB test results
