Job description

About us: 3rd Street Youth Center & Clinic is a community-based agency providing youth in the Bayview Hunters Point with medical and behavioral health services that encourage them to make decisions that support their health, safety, and development. 3rd Street Youth Center & Clinic also manages a number of successful Transitional Age Youth (TAY) housing stabilization programs and one of the city's few Youth Access Points.
We consider health education to be a vital tool in promoting health behaviors and preventing disease in our youth. The Health & Youth Development Team leads our young people through engaging lessons and open discussions about physical, mental, social and sexual health. Through our Youth Clinic, young patients can seek confidential care and coordination of medical services. Our Youth Development programs educate young people about social determinants of health, community outreach, civic engagement, and health professions.
Program info: The Health Core Fellowship Program is a one-year fellowship program that prepares young adults ages 17 - 24 to become certified Community Health Workers (CHWs). The program explores health topics such as chronic disease, infectious disease, trauma, and sexual health with a lens on prevention while also building the core competencies of the CHW role, such as health education, community outreach, care coordination, etc. Over the course of the year, participants complete a phased sequence of coursework culminating in a CHW certificate from City College of San Francisco, a paid CHW internship, and a direct pathway into community health employment.
Position summary: Reporting directly to the Director of Health & Youth Development, the Health Core Fellowship Program Coordinator II plans and facilitates the full cycle of the Health Core Fellowship Program - from curriculum design and youth facilitation to the external partnerships (City College San Francisco, hospitals, clinics, etc.) that make certifications, mentorship, and paid internships possible.
