Overview:
Our client, is pioneering a new primary care model for patients with End-Stage Renal Disease (ESKD), integrated directly into their dialysis centers. They are seeking a highly experienced and innovative Nurse Practitioner (NP) to serve as the founding clinician of this groundbreaking program. This is a Monday–Friday onsite role based in the Chesapeake/Norfolk, VA area.

Key Responsibilities:

  • Provide comprehensive primary care for patients with complex chronic conditions, particularly ESKD.

  • Collaborate with multidisciplinary care teams within dialysis centers.

  • Prescribe and manage medications following evidence-based guidelines.

  • Support the development and launch of a new clinical program focused on patient-centered outcomes.

  • Track and achieve performance metrics aligned with clinical and operational excellence.

Qualifications:

  • Active NP License to practice autonomously in Virginia.

  • Minimum 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.

  • Expertise managing chronic diseases such as diabetes, hypertension, COPD, and heart disease.

  • DEA license with comfort in prescribing for complex chronic conditions.

  • Must live locally to Chesapeake/Norfolk, VA.

Preferred Experience:

  • Background in dialysis or nephrology (preferred but not required).

  • Experience in program development or clinical innovation.

  • Success working in performance-driven care models.

Compensation & Benefits:

  • Base salary: $130K – $170K (depending on experience).
