Are you an LVN who wants to make a meaningful difference through one-on-one patient care? At ComForCare Home Care, our nurses provide compassionate, high-quality care that helps patients remain safe, comfortable, and supported in their homes.
We are looking for dependable, skilled, and patient-focused Licensed Vocational Nurses to join our growing home health team. This role is a great fit for LVNs who enjoy working independently while still being supported by an experienced care team.
What You'll Do
  • Provide direct nursing care in patients' homes according to the plan of care
  • Administer medications, injections, treatments, and wound care as ordered
  • Monitor vital signs, symptoms, and patient progress
  • Communicate changes in condition to the RN, physician, and care team as appropriate
  • Coordinate care with the interdisciplinary team
  • Complete accurate and timely documentation, including EMR documentation
  • Follow infection control, safety, HIPAA, and agency compliance standards
  • Support personal care needs within LVN scope of practice when appropriate

What We're Looking For
  • Current California LVN license
  • Graduate of an accredited nursing program
  • Current CPR certification
  • Home health experience preferred
  • Wound care and venipuncture experience preferred
  • Comfortable with EMR documentation
  • Strong communication, observation, and time-management skills
  • Ability to work independently and coordinate with a care team
  • Valid California driver's license, auto insurance, and reliable transportation
