TruBlu HR Solutions is seeking a qualified Social Worker for a contract opportunity with a leading pediatric healthcare organization in The Woodlands, TX. This position is ideal for a compassionate and experienced social work professional who is skilled in psychosocial assessment, crisis intervention, care coordination, and supporting patients and families through complex situations.

Position Details

Job Title: Social Worker
Location: The Woodlands, TX 77384
Start Date: August 31, 2026
Schedule: Monday–Friday, 7:30 AM–4:00 PM
Shift: Day Shift
Contract: Approximately 4 months
Potential: Contract may be extended or converted to a permanent position based on performance

Job Responsibilities

  • Conduct comprehensive psychosocial assessments and evaluate patient and family risk factors.
  • Develop, implement, and evaluate individualized plans of care.
  • Communicate recommendations effectively with patients, families, physicians, and interdisciplinary healthcare teams.
  • Coordinate resources and interventions based on social, emotional, physical, and environmental factors affecting patient care.
  • Identify barriers to care and discharge and assist patients and families with appropriate resources.
  • Provide support during crisis situations, grief, bereavement, and end-of-life care.
  • Support families experiencing complex family dynamics and assist with de-escalation when appropriate.
  • Assist with legally complex cases by connecting patients and families with appropriate community resources.
  • Advocate for patients and families while maintaining a family-centered approach to care.
  • Maintain patient privacy and confidentiality in accordance with HIPAA and healthcare regulations.
  • Accurately document assessments, interventions, recommendations, and care plans in the medical record.

Qualifications

  • Master’s Degree in Social Work (MSW)
  • Active Texas social work license – Required: LMSW, LCSW, or LMSW-AP
  • Strong experience conducting psychosocial assessments and developing plans of care.
  • Ability to manage crisis situations and complex psychosocial needs.
  • Excellent critical thinking, judgment, problem-solving, and decision-making skills.
  • Strong professional communication and active listening skills.
  • Ability to coordinate community resources and advocate effectively for patients and families.

Preferred Qualifications:

  • Hospital or healthcare social work experience
  • Pediatric social work experience
  • Behavioral health experience
  • Experience working with interdisciplinary healthcare teams
