Clinic Nurse
Carol Woods Retirement Community- Primary Care Clinic
We are a Primary Care Clinic located inside of Carol Woods Retirement Community seeking a Clinic Nurse who is self-motivated, dependable, and a friendly team member. Someone who takes pride in their work, wants to provide high-quality service, and enjoys working with older adults.
Roles and Responsibilities:
  • Performs all nursing functions, including clinic triage, home visits, and annual health updates.
  • Assists with basic clerical operations within the clinic.
  • Communicates effectively with the providers to carry out physical assessment, medical orders, administer prescription medications, and perform treatments.
  • Instructs residents, caregivers, and staff in health care protocols.
  • Documents services, care provided, and providers orders.
  • Performs phlebotomy, EKG's, lab procedures, dressing changes, etc.
  • Communicates effectively with family and health care team members to provide continuity in health care.
  • Complies with all infection control requirements.
  • Ensures that written and verbal reports are given daily to the House Call Nurse covering emergent and urgent visits during evenings and on weekends in addition to arranging follow-up care.
  • Responds to medical emergencies in the home or other areas of Carol Woods and makes appropriate referrals upon consultation with provider leadership, as available.
  • Evaluates, obtains medical orders, and provides appropriate nursing care to residents in the home setting.
  • Assesses and monitors resident's ability to comply with medication/treatment regimens and safety standards in the home setting.
  • Makes appropriate referrals for in-home services and coordinates and oversees care provided in the home according to Carol Woods.
  • Provides information for resident level of care decisions and transitions to appropriate personnel (e.g., Social Worker, Resident Life Coordinator, Director of Nursing, etc.).
  • Completes lab requisitions and other documentation with ICD-10 coding.

When the Clinic RN serves as a back-up to the Triage Nurse, in addition to the above, the following are included:
  • Communicates with clinic staff and manager to adjust workload as needed.
  • Facilitates the workflow of all health care providers in a well-organized manner.
  • Reviews morning report from Assisted Living and Health Center night calls with Director, CWs Clinic, and others.
  • Orders medication refills and obtains authorization from providers as needed.
  • Assesses, triages, and provides nursing care to residents appropriately and in an organized and efficient manner; consults with providers, as needed.
  • Communicates with the clinic scheduler to schedule residents appropriately.

All other duties as assigned
Education:
  • Graduation from an accredited school of nursing.
  • Current unencumbered license as a Registered Nurse in North Carolina or able to obtain a North Carolina license (NC license must be granted prior to hiring).
  • Current Adult CPR certification.

Experience:
Minimum of two years experience preferably with older adults patients and ambulatory care services. Previous knowledge of Epic preferred.
Physical Requirements:
  • Able to bend, stoop, reach, and lift to assist residents.
  • Able to stand and walk for long periods and to move equipment and small pieces of furniture.
  • Able to perform CPR.
